Refer A Patient
We are happy to accept referrals as follows:
BY TELEPHONE
Please telephone us on 0161 320 4230 with your patient's details and an outline of the case. We will contact your patient and arrange an appointment. Please give the patient a referral letter and any radiographs etc to bring with them to their appointment. Alternatively send these items in the post.

When we receive your message we will contact your patient and arrange an appointment. If you need to send radiographs etc, please give these to your patient to bring with them to their appointment or send them by post. If you have digital radiography you can also e-mail radiographs to us.

REFERRAL LETTERS
There are specific downloadable referral forms on the relevant pages of this site. For example there is a referral form for endodontics, one for restorative dentistry, etc. Please use these forms if it makes it easier to refer patients to us. The referral forms are also on the Documents & Links page
If you find it easier to write your own letter then please do so.
In most cases please include the following information in your letter:
• The date of referral
• Your own name, address and telephone number
• The patients name, address and telephone number
• Any relevant medical history
• Special need or requests
• The condition initiating referral
• Is the referral for treatment, advice or a 2nd opinion?
• Is the referral urgent?
• Include results of any special tests, relevant radiographs, study casts etc.
Where possible we suggest that you keep a copy of your referral letter in your own patient records.
